We undertake to begin care of patients within 24 hours according to the procedure below:

Request for
admission

Made by the patient, their family, their GP, hospitals, CTR or other external partners.

2.

Assessment of
the situation

1st assessment at home by a nurse.

Analysis of information.

Request for additional information from GP, health partners and relatives.

3.

Admission

A patient case file and RAI assessment are created.

Service contract.

Care plan finalised.

Services begin.

4.

Invoicing

Soins Riviera contacts the GP requesting his or her signature for the care request. Soins Riviera sends a monthly invoice to the patient’s health insurance company.
